Whether you book your first appointment online or with us over the phone, you should get an automated email from Massagebook with a link to fill out something called an Intake Form. This form helps provide our therapists with the medically relevant (or even necessary) information they'll need in order to best serve you. It is stored very securely in a convenient digital format for our therapists' reference.

As most of our work is done in a medical context, we're frequently asked if we accept insurance. We are not credentialed with any insurance company, nor do we work directly with any carrier ourselves. However, some current and past clients of ours have been able to work with their healthcare provider and insurance company in order to be reimbursed after the fact. Below, you'll find a link to a form that may aid you in your reimbursement-related endeavors; however, while we wish you success, we cannot make any guarantees on behalf of your insurance carrier.
